The investigation was launched in response to a request from the Prime Minister in the wake of the Stafford Hospital scandal.

Sir Bruce said: "These hospitals are already working closely with a range of regulators.

"If there were concerns that services were unsafe the regulators should have intervened.

"The purpose of my investigation is to assure patients, public and Parliament that these hospitals understand why they have a high mortality and have all the support they need to improve.

"This will be a thorough and rigorous process, involving patients, clinicians, regulators and local organisations."

The five trusts announced on February 6 have been "outliers" for two years on the Summary Hospital-level Mortality Indicator (SHMI).

And the nine organisations announced today have been "outliers" for two years on the Hospital Standardised Mortality Ratio (HSMR).

The trust said its mortality rates had improved in the last year although admitted there was "still more work to do".

A statement said: "We welcome the investigation into mortality indicators within our trust and hope that it will provide an opportunity to demonstrate the progress that we have made on mortality thanks to the hard work of our staff.

"Our SHMI continues to fall from a peak of 133 in quarter 3 2009/10.

"In the last available quarter (to June 2012), the Trust was no longer an outlier, with a quarterly SHMI of 106.28. Our HSMR figure has also shown recent improvement.

"Additionally, we have made significant improvements in all areas of patient safety."

The other trusts announced today were:

  • North Cumbria University Hospitals NHS Trust
  • George Eliot Hospital NHS Trust
  • Buckinghamshire Healthcare NHS Trust
  • The Dudley Group NHS Foundation Trust
  • Sherwood Forest Hospitals NHS Foundation Trust
  • Medway NHS Foundation Trust
  • Burton Hospitals NHS Foundation Trust

“My mum was admitted to Grimsby hospital with what appeared to be stroke like symptoms. A CT scan was given and the doctor in A&E looked at it and seeing a dark area in one of the cerebral hemispheres, concluded that it was a stroke caused by a bleed. With my little knowledge on the subject and 10 minutes of internet research suggested that he was wrong, as a bleed shows up as white on CT scans. I asked him if he was sure and asked for a second opinion. The scans were emailed to a neurologist and he said that there was no bleed, and the dark area was normal aging of the brain (she is nearly 80 years old). Then they went on to say that it was a urinary tract infection. Then when she recovered quite quickly, they discharged her saying that it was a pulmonary (chest) infection. They never gave her a second CT scan as I asked; as if it was a stroke or transient ischemic attack (mini stroke) it takes some days for the damage to show itself on a CT scan. This was all just before Christmas. Well the same thing has happened again to my mum this week. This time she is in Lincoln County. I tried to tell them that this same thing happened before and they wouldn't listen. They seem to be of the attitude that she is old and therefore not worth their attention. Appalling attitude and ineptitude.”

“What is so daft about the investigation is that the trust hold all the records to prove it, so will the records be lost or accidently misplaced when the new computer system collapses, and the doctors will stick together as will the nurses, this will be one big loss of time and money, they have the right to deny and lie, and what really makes it worse, is the good ones made to keep quiet or face the sack. It's a NHS LOT,”
